This questionnaire was distributed to emigrants from Nazi-occupied Europe who arrived in the US. It was created by the Committee for the Study of Recent Immigration from Europe as a fact-finding mission on the immigration wave that began with the ascendancy of the Nazis in Germany in 1933. It was sponsored by the American Christian Committee for Refugees, the American Friends Service Committee, the National Refugee Service, and the United States Committee for the Care of European Children. Questions on the form pertain to a variety of topics, including country of origin, reason for immigration, religion, and whether the immigrant would like to return to Europe.
